Regarding requests for disclosure of held personal information

Those who can request disclosure

Anyone can request the disclosure of personal information held about themselves.
A representative of a minor or an adult ward, or a representative delegated by the person in question (voluntary representative), may request the disclosure of held personal information on behalf of the person in question.

Information subject to disclosure request

This applies to personal information held.

(Note) Personal information held
This refers to personal information that has been created or obtained by officers or employees of Incorporated Administrative Agency, etc. in the course of their duties, that is held by the Incorporated Administrative Agency, etc. as information for organizational use by the officers or employees, etc. of the Incorporated Administrative Agency, etc., and that is recorded in corporate documents as stipulated in Article 2, Paragraph 2 of the Act on Access to Information Held by Incorporated Administrative Agency etc.

Personal information held, excluding non-disclosure information

(Note) Examples of non-disclosure information
- Information that may be harmful to the life, health, lifestyle or property of the person requesting disclosure
- Information about an individual other than the requester that can identify a specific individual, such as name or date of birth, or information whose disclosure may harm the rights and interests of an individual
- Information that may harm the rights or interests of corporations or other organizations
- Information concerning deliberations, examinations, etc. that is likely to unduly impair the frank exchange of opinions or to unduly cause confusion among the public
- Information that impedes the proper execution of the affairs and business of Incorporated Administrative Agency, etc.

Procedure for requesting disclosure

Inquiries, reception and consultation regarding disclosure requests
We will identify the information to be disclosed (personal information held).
We will endeavor to identify the information required by the requester and to resolve any misunderstandings with the requester.

Please fill out the necessary information on the Personal Information Disclosure Request Form and submit it. You can also submit it by mail. (Please note that submissions by e-mail or fax are not accepted.)

Amendment of disclosure request
This will be necessary if there are any deficiencies in the request form for disclosure of held personal information.
Please note that the time required to make corrections will not be counted toward the 30-day deadline for making a decision to disclose or not disclose.

(Note) When a corporation receives a request for disclosure of personal information it holds, it must, in principle, make a decision as to whether to disclose or not disclose the information within 30 days.

Please note that, in principle, applications for disclosure of held personal information must be submitted within 30 days of the date on which the decision to disclose is made.
