Notification of address change Notification of surname (first name) change Notification of change of place of residence	Applications for changes to registration information have been changed to be made on SIRIUS. Please apply from "Menu" on the left side of the SIRIUS screen → "Various applications/links" → "Various applications" → "New application" → Change of registered information (self) / Report of change of address / Phone number (for international students).
Notification of change of guarantor	The Guarantor Change Notification form can be obtained at the Student Support Section Section.

The "Off-Campus Research Notification" application has been changed to an application on SIRIUS. "Menu" on the left side of the SIRIUS screen→ "Applications/Links" →"Various Applications" →"New Applications"→ Notification of External Research (Faculty of Engineering and Graduate School of Engineering ・BASE) *Please apply from Notification of Extramural Research For Assigned Lab Students (4th year undergraduate students, graduate students) assigned to the laboratory.

The application for the "Research Topic Notification" and "Research Guidance Plan" has been changed to be done via SIRIUS. Please apply from "Menu" on the left side of the SIRIUS screen → "Various Applications/Links" → "Various Applications" → "New Application" → [BASE] "Research Title Notification" and "Research Guidance Plan". After that, your supervisor will be asked to complete and approve your research plan.

The application for the "Research Topic Notification" and "Research Guidance Plan" has been changed to be done via SIRIUS.

Please apply from "Menu" on the left side of the SIRIUS screen → "Various Applications/Links" → "Various Applications" → "New Application" → [BASE] "Research Title Notification" and "Research Guidance Plan".
After that, your supervisor will be asked to complete and approve your research plan.
